New recall system for long term conditions

As of the 1st April we are planning to change our recall system for patients who have long-term conditions, for example, diabetes, COPD, Asthma, hypertension etc. Our plan is to call patients for their reviews during their birthday month. We are planning to streamline the process by requesting all blood tests during the patients birthday month followed up by an invitation to make an appointment with the relevant clinician for the review/s. Please bear with us as we introduce this new system. The plan is to reduce the number of appointments needed to aid efficiency, free up appointments and provide a more holistic model for patient care . Obviously there will be times where clinicians will request additional tests but for the main part we will try to order everything together.  If you have any queries or feedback, please do let us know.
